The Courthouse Athletic Center Cancellation Policy

Due to the amount of planning necessary to run our programs, if you intend to cancel you must do so no later than 72 hours prior to the start of the event in order to receive a partial refund. Any cancellations made later than this will NOT be refunded.

Participant / Parental Permission

ALL YOUTH PARTICIPANTS MUST HAVE A COMPLETED WAIVER AND RELEASE FORM SIGNED BY HIS/HER PARENT OR LEGAL GUARDIAN PRIOR TO PARTICIPATING IN ANY PROGRAMS AT THE COURTHOUSE ATHLETIC CENTER (for those under the age of 18).

PLEASE NOTE: This form must be completed by all participants prior to start of any program at The Courthouse Athletic Center.

I am completely in favor of my child participating in programs at The Courthouse Athletic Center. I agree that sports are by nature rough and that the risk of injury can never entirely be eliminated.

I have read the waiver below and take full responsibility for this person's participation and hold The Courthouse Athletic Center harmless from any injury, loss, or damage.

There is a risk: I know that these programs will expose me to above the normal risk of injury or harm. Physical contact or collisions with other players and or objects are hazards that I might encounter. I take full responsibility for any damage or harm that might come to me or that I might cause to someone else.

I am healthy enough to play: I have no health or physical problems that might happen to get worse with the intense exertion and physical contact of sports or physical activity. I am solely responsible for my own safety. I take responsibility for monitoring my own physical conditions, risks, and limitations. I make my own judgments about when, how often, and how intensely I participate.

I release The Courthouse Athletic Center from responsibility: I fully release The Courthouse Athletic Center from any and all claims, causes of action, liability for injuries, and losses or damages that might arise from or be in any way associated with my participation or attendance in activities. The waiver additionally agrees to hold harmless The Courthouse Staff and all sponsors from all risks mentioned or not mentioned in this agreement, as well as all claims and or causes of action or liability arising out of the negligence of The Courthouse staff.

I agree to indemnify and hold harmless The Courthouse Staff from any loss, damage, or expense sustained or incurred by them arising from such claims and or causes of action or liability. This includes legal action brought to me, someone acting on my behalf, or anyone else because of the conduct attributed to me.

I agree that this agreement shall be binding upon my heirs, assigns, and any personal entity acting on my behalf. This includes: parents, guardians, relatives, and or friends.

I have read and understand this waiver. I agree to abide by all responsibilities and conditions it includes.
